Before you begin

Please review the following Terms carefully. By checking the box and electronically signing below, you agree to participate in the DiaFem Inc. Whole-Body Analysis Beta Program.

What you're agreeing to

DiaFem Inc. is conducting a limited beta program with up to 50 participants. As part of the program, you will complete a comprehensive assessment covering approximately 140 points of health and wellness information.

Based on the information you provide, DiaFem Inc. will prepare an educational Whole-Body Analysis designed to organize your health information and identify patterns and relationships within the information you provide.

The Analysis is educational and informational only. It is not a diagnosis, medical examination, individualized clinical evaluation, medical advice, or treatment recommendation.

Licensed clinician review

Before your Analysis is provided to you, a licensed clinician may review the Analysis and the underlying information you provided as part of DiaFem's clinical oversight and review process.

The clinician may review, edit, approve, modify, or remove portions of the Analysis.

This review is intended to help ensure that the Analysis is appropriately supported by the information provided, reasonable within the educational scope of the service, and presented clearly and responsibly.

This clinician review is not a clinical visit. The reviewing clinician does not examine you, diagnose you, prescribe treatment, provide individualized medical advice, or establish a clinician-patient relationship with you as part of this review.

Separate clinical care

After receiving your Analysis, you may separately choose to schedule a clinical visit with an independent licensed clinician.

That is a separate and optional workflow. Any diagnosis, medical evaluation, treatment recommendation, prescription, or treatment protocol would occur only during that separate clinical relationship.

You are not required to schedule a clinical visit.

Your health information

You understand that DiaFem Inc. will collect and use the health information you provide to provide your Analysis and administer the Beta Program.

DiaFem Inc. does not sell your personal information or consumer health data.

Important limitations

The Analysis is based on the information you provide and may be incomplete or contain errors. It may not identify a medical condition or explain the cause of a symptom.

Do not start, stop, or change medication, hormone therapy, supplements, or other medical treatment based solely on your DiaFem Analysis.

DiaFem Inc. does not provide emergency or urgent medical care. If you are experiencing an emergency, call 911 or seek immediate medical attention.

Beta participation

The Beta Program is free. There is no obligation to purchase anything or schedule a clinical visit.

Because this is a beta program, DiaFem Inc. may modify, improve, or discontinue the assessment, Analysis, or related services.
